Bone Health Supplements: How Calcium, Vitamin D, and Vitamin K Compare
A plain-language look at what the evidence says about three widely used bone health nutrients, based on data from the NIH Office of Dietary Supplements.

Calcium, vitamin D, and vitamin K each play distinct roles in keeping bones strong, and no single supplement does the whole job. Understanding what the research actually supports, and where the evidence is still limited, can help you have a more informed conversation with your doctor or pharmacist.

Calcium: The Structural Foundation
What it does
According to the NIH Office of Dietary Supplements (ODS), calcium makes up much of the structure of bones and teeth, keeping tissue rigid, strong, and flexible. It also mediates blood vessel contraction and dilation and supports normal muscle function.
How much do adults need?
The ODS sets the adult Recommended Dietary Allowance at 1,000 mg per day for adults aged 19 to 50. The target rises to 1,200 mg per day for women after age 50 and for men after age 70. The Tolerable Upper Intake Level (UL) is 2,500 mg per day for adults aged 19 to 50, and 2,000 mg per day for adults aged 51 and older, counting all sources combined.
What the evidence shows
- Strong: Calcium is required for normal bone and tooth structure, muscle function, nerve transmission, blood clotting, and blood vessel function.
- Moderate: Supplemental calcium, alone or with vitamin D, increases bone mineral density in older adults.
- Limited: Calcium plus vitamin D supplementation reduces the risk of fractures in older adults. There is also limited evidence that calcium supplements may increase the risk of kidney stones or cardiovascular disease in some people.
- Insufficient: Higher calcium intake lowers the risk of colorectal or other cancers.
Safety and practical notes
For most healthy people, calcium from food is safe. Absorption is best at single doses of 500 mg or less, and taking supplements with food can improve uptake. Staying below the UL helps avoid the risk of kidney stones and possible cardiovascular concerns.
Drug interactions to know
- Dolutegravir (HIV integrase inhibitor): Calcium can substantially reduce blood levels of this drug through chelation. Take dolutegravir 2 hours before or 6 hours after calcium supplements.
- Levothyroxine (thyroid hormone): Calcium carbonate can interfere with absorption. Separate doses by at least 4 hours.
- Lithium (bipolar disorder): Long-term use can cause high blood calcium, and combining with calcium supplements may increase that risk.
- Quinolone antibiotics (ciprofloxacin, gemifloxacin, moxifloxacin): Calcium reduces antibiotic absorption. Take the antibiotic 2 hours before or 2 hours after calcium supplements.
Vitamin D: The Absorption Enabler
What it does
The ODS describes vitamin D as the nutrient that promotes calcium absorption in the gut and maintains adequate calcium and phosphate levels in the blood to enable normal bone mineralization. It also supports bone growth and remodeling by osteoblasts and osteoclasts, and helps protect older adults from osteoporosis when combined with adequate calcium intake.
How much do adults need?
The ODS target is 15 mcg (600 IU) per day for adults aged 19 to 70, rising to 20 mcg (800 IU) for adults aged 71 and older. The UL from all sources, including food, beverages, and supplements, is 100 mcg (4,000 IU) per day for everyone aged 9 and older, including during pregnancy and lactation. A healthcare provider may prescribe higher doses in specific clinical situations.
What the evidence shows
- Strong: Vitamin D promotes intestinal calcium absorption and maintains the blood levels of calcium and phosphate needed for normal bone mineralization. Vitamin D deficiency causes rickets in children and osteomalacia in adults.
- Moderate: Vitamin D plus calcium supplements slightly increase bone mineral density in some postmenopausal women and older men, and can reduce fracture rates in institutionalized older people.
- Limited: Vitamin D supplements reduce falls or fractures in community-dwelling older adults.
- Insufficient: Vitamin D supplements prevent or treat cancer, cardiovascular disease, depression, multiple sclerosis, or type 2 diabetes, or aid weight loss.
Drug interactions to know
- Orlistat (Xenical, Alli): This weight-loss drug can reduce absorption of vitamin D from food and supplements.
- Statins (atorvastatin, lovastatin, simvastatin): High-dose vitamin D supplements may reduce how well these medications work.
- Corticosteroids such as prednisone: Can lower blood levels of vitamin D.
- Thiazide diuretics (chlorthalidone, hydrochlorothiazide): Combined with vitamin D supplements, these can raise blood calcium to unsafe levels.
Vitamin K: The Bone Protein Activator
What it does
Vitamin K acts as a coenzyme for vitamin K-dependent carboxylase, the enzyme required to synthesize proteins involved in blood clotting, including prothrombin (clotting factor II). It also supports the carboxylation of osteocalcin, a vitamin K-dependent protein in bone that may be involved in bone mineralization or turnover.
How much do adults need?
The ODS sets an Adequate Intake (AI) rather than an RDA, because data are not sufficient to establish a firm requirement. The AI is 120 mcg per day for men and 90 mcg per day for women. No Tolerable Upper Intake Level has been established, reflecting a low potential for toxicity from food or typical supplement amounts.
What the evidence shows
- Strong: Vitamin K functions as a coenzyme for vitamin K-dependent carboxylase and is required for synthesis of clotting proteins such as prothrombin. Vitamin K also supports carboxylation of osteocalcin and other bone proteins and is involved in bone metabolism.
- Moderate: Gut bacteria-produced menaquinones (a form of vitamin K) contribute to meeting the body's vitamin K requirement.
- Limited: Vitamin K supplementation reduces the risk of osteoporosis or bone fracture. Higher menaquinone (K2) intake is associated with less vascular calcification and lower coronary heart disease risk.
Drug interactions to know
- Warfarin (Coumadin) and similar anticoagulants: This is the most important safety consideration. Vitamin K antagonizes these drugs, and sudden changes in vitamin K intake from food or supplements can increase or decrease the anticoagulant effect. People on warfarin must keep their vitamin K intake consistent and should not change supplement use without medical guidance.
- Antibiotics: Can reduce vitamin K-producing gut bacteria. Cephalosporins such as cefoperazone may also inhibit vitamin K action directly.
- Bile acid sequestrants (cholestyramine, colestipol): Reduce absorption of vitamin K and other fat-soluble vitamins with long-term use.
- Orlistat: Reduces fat absorption and can lower absorption of fat-soluble vitamins including vitamin K, and when combined with warfarin may significantly increase prothrombin time.
How These Three Nutrients Work Together
Calcium provides the raw material for bone tissue. Vitamin D ensures that calcium is absorbed from the digestive tract and that blood levels stay high enough to support ongoing bone maintenance. Vitamin K activates the proteins that incorporate calcium into bone and regulate how the body handles it. A shortfall in any one of the three can limit the effectiveness of the others.
The Bottom Line
All three nutrients have strong evidence supporting their basic physiological roles in bone metabolism, according to the NIH ODS. Evidence that supplements reduce fracture risk is moderate to limited and varies by population, particularly for older or institutionalized adults. Evidence for broader disease prevention claims remains insufficient. Because optimal intake depends on age, sex, diet, health status, and medications, consult a clinician or pharmacist before starting or adjusting any supplement regimen, and always talk to your doctor before combining supplements with prescription or over-the-counter medications.
