Vitamin D for Breastfed Infants and Nursing Mothers
A practical, evidence-based overview of vitamin D needs during breastfeeding, what the research actually shows, and when to talk to your doctor.

Breast milk is the gold standard for infant nutrition, yet it typically provides very little vitamin D, making supplementation a common topic for new families. Understanding the physiological role of vitamin D, the targets set by the NIH Office of Dietary Supplements (ODS), and the limits of current evidence can help you have a more informed conversation with your baby's pediatrician and your own healthcare provider.
What Vitamin D Does in the Body
According to the NIH ODS, vitamin D promotes calcium absorption in the gut and helps maintain adequate serum calcium and phosphate concentrations. These functions are essential for normal bone mineralization, bone growth, and the ongoing remodeling carried out by osteoblasts and osteoclasts. Vitamin D also helps prevent hypocalcemic tetany, a condition involving involuntary muscle contractions, cramps, and spasms that can occur when blood calcium falls too low.
The evidence that vitamin D supports these bone and calcium functions is graded strong by the NIH ODS. Vitamin D deficiency is strongly linked to rickets in children and osteomalacia in adults. Beyond bone health, research into other proposed benefits, including effects on cancer, cardiovascular disease, depression, or type 2 diabetes, is currently graded insufficient by the NIH ODS, meaning the data do not yet support firm conclusions.
Daily Targets and Upper Limits
The NIH ODS Dietary Reference Intake figures for vitamin D are as follows:
- Infants (0 to 12 months): 10 mcg (400 IU) per day is the Adequate Intake. The Tolerable Upper Intake Level (UL) is 25 mcg (1,000 IU) per day for infants up to 6 months, and 37.5 mcg (1,500 IU) per day from 7 to 12 months.
- Adults including nursing mothers (19 to 70 years): 15 mcg (600 IU) per day is the Recommended Dietary Allowance. The UL from all sources, including food, beverages, and supplements, is 100 mcg (4,000 IU) per day.
Staying within the UL matters because excessive vitamin D from supplements can raise blood calcium to unsafe levels, a condition called hypercalcemia. A healthcare provider may prescribe higher doses in specific clinical situations, but self-directed supplementation above the UL is not recommended.
Why Breastfed Infants Are at Risk
Human breast milk contains very little vitamin D, generally well below the infant Adequate Intake of 10 mcg (400 IU) per day, regardless of the mother's own vitamin D status. Because of this, major pediatric and public health organizations recommend that exclusively and partially breastfed infants receive a daily vitamin D supplement starting shortly after birth. Formula-fed infants who consume at least one liter of fortified formula per day typically meet their needs through formula alone, but this is worth confirming with a pediatrician.
Skin synthesis of vitamin D from sunlight is an alternative source, but safe sun exposure for newborns and young infants is difficult to define and generally not recommended as a primary strategy due to skin sensitivity and UV risk.
What About the Nursing Mother?
A nursing mother's own vitamin D needs do not increase above the standard adult Recommended Dietary Allowance of 15 mcg (600 IU) per day, according to the NIH ODS. The UL of 100 mcg (4,000 IU) per day applies during lactation just as it does at other life stages.
Some research has explored whether giving nursing mothers very high doses of vitamin D, well above standard recommendations, could raise breast milk levels enough to meet an infant's needs without a separate infant supplement. The NIH ODS notes this area is still being studied, and at this time the standard guidance remains to supplement the infant directly. Do not adjust your own dosing based on this concept without speaking to a clinician first.
Drug Interactions to Know
If you are a nursing mother taking any medications, it is important to talk to your doctor or pharmacist before adding a vitamin D supplement. The NIH ODS identifies the following interactions:
- Orlistat (Xenical, alli): This weight-loss drug can reduce absorption of vitamin D from food and supplements.
- Statins (for example atorvastatin/Lipitor, lovastatin/Altoprev, simvastatin/Zocor): High-dose vitamin D supplements may reduce how well these cholesterol medications work.
- Corticosteroids such as prednisone (Deltasone, Rayos, Sterapred): These can lower blood levels of vitamin D over time.
- Thiazide diuretics (for example hydrochlorothiazide/Microzide, chlorthalidone/Hygroton): When combined with vitamin D supplements, these medications can raise blood calcium to potentially harmful levels.
This list is not exhaustive. Always disclose all supplements to your healthcare team.
Choosing and Using a Supplement
For infants, liquid vitamin D drops formulated specifically for babies are the most practical option. Check the label carefully, as concentrations vary between products and an accidental overdose is possible if the dropper is misread. For nursing mothers, a standard multivitamin or a dedicated vitamin D supplement at or near the 15 mcg (600 IU) daily target is generally sufficient for maintaining personal vitamin D status.
Vitamin D supplements are not intended to diagnose, treat, cure, or prevent any disease, and these statements have not been evaluated by the FDA.
When to See a Clinician
Vitamin D needs can vary based on skin tone, geographic location, time spent outdoors, body weight, and certain medical conditions. A blood test measuring serum 25-hydroxyvitamin D is the standard way to assess status. The following situations warrant a conversation with a doctor or pharmacist:
- Your infant has a medical condition affecting nutrient absorption.
- You are taking any of the medications listed above.
- You are considering a dose above the standard daily recommendation.
- Your infant is not gaining weight as expected or shows signs of bone tenderness.
Personal dosing decisions should always be made with a qualified clinician, not based on population reference values alone.
