Calcium citrate does not depend on stomach acid to the same degree. The trade-off is that tablets tend to be larger or more numerous because of the lower elemental calcium percentage per gram.

The NIH ODS notes that absorption is best when calcium is taken in single doses of 500 mg or less, and that taking supplements with food improves absorption for carbonate forms. Practical guidance based on those findings:
- Calcium carbonate: take with a meal or snack to maximize dissolution and absorption.
- Calcium citrate: can be taken with or without food, giving more scheduling flexibility.
- Regardless of form, split doses across two or more meals if your total supplemental dose exceeds 500 mg of elemental calcium per sitting.
What the Evidence Says About Calcium Supplementation

- Lithium (used for bipolar disorder): long-term use can cause elevated blood calcium, and combining lithium with calcium supplements may increase that risk.
- Quinolone antibiotics (ciprofloxacin, gemifloxacin, moxifloxacin): calcium reduces antibiotic absorption. Take the antibiotic 2 hours before or 2 hours after calcium supplements.

For most healthy people, calcium from food is safe at normal dietary levels, according to the NIH ODS.
