Calcium Deficiency: Bone Loss, Muscle Cramps, and Hidden Long-Term Risk
Calcium is the most abundant mineral in the body, and falling short of daily targets can quietly affect bones, muscles, and more over time.

Signs That Intake May Be Low
Mild, chronic low calcium intake rarely causes dramatic symptoms right away because the body draws calcium from bones to keep blood levels stable. Over years, that withdrawal weakens bone density. More noticeable short-term signs of very low calcium, known as hypocalcemia, can include muscle cramps, tingling in the hands and feet, and in severe cases abnormal heart rhythms. These symptoms require medical evaluation and are not a reason to self-prescribe high-dose supplements.
What the Research Shows About Supplements
It is important to understand that calcium supplements are not intended to diagnose, treat, cure, or prevent any disease, and these statements have not been evaluated by the FDA. Research does show physiological effects, but the strength of that evidence varies considerably.
Bone Mineral Density
The NIH rates the evidence as moderate that supplemental calcium, alone or combined with vitamin D, increases bone mineral density in older adults. Higher density is generally associated with stronger bones, though density is only one factor in fracture risk.
Fracture Risk
Evidence that calcium plus vitamin D supplementation reduces fracture risk in older adults is rated as limited by the NIH. This means some studies show a benefit, but the overall body of research is not yet consistent enough to draw firm conclusions.
Cancer Risk
Some research has explored whether higher calcium intake lowers the risk of colorectal or other cancers. The NIH grades this evidence as insufficient, meaning current data do not support a reliable conclusion either way.
Cardiovascular Risk and Kidney Stones
The NIH also notes limited evidence that calcium supplements may increase the risk of cardiovascular disease or kidney stones. This concern applies primarily to supplemental calcium rather than calcium from food. Adults should stay below the Tolerable Upper Intake Level, which is 2,500 mg per day for those aged 19 to 50 and 2,000 mg per day for those aged 51 and older, counting all sources combined.
Getting Calcium From Food First
For most healthy people, calcium from food is safe and comes packaged with other nutrients that support absorption. Good dietary sources include dairy products, fortified plant milks, canned fish with soft bones, tofu made with calcium sulfate, and leafy greens such as kale and bok choy. Prioritizing food sources before turning to supplements is a reasonable starting point for most people.
If You Do Use Supplements
Absorption of supplemental calcium is best when taken in single doses of 500 mg or less, and taking supplements with food improves absorption further, according to the NIH Office of Dietary Supplements. Splitting a larger daily dose into two smaller doses across the day is more effective than taking it all at once.
The two most common forms are calcium carbonate, which requires stomach acid and is best taken with meals, and calcium citrate, which can be taken with or without food and may suit people with lower stomach acid.
Drug Interactions You Should Know About
Calcium supplements interact with several medications. Always talk to a doctor or pharmacist before combining supplements with any prescription or over-the-counter drug. Key interactions identified by the NIH include:
- Dolutegravir (HIV integrase inhibitor): Calcium can substantially reduce blood levels of this medication through chelation. Take dolutegravir 2 hours before or 6 hours after calcium supplements.
- Levothyroxine (thyroid hormone): Calcium carbonate can interfere with levothyroxine absorption. Separate doses by at least 4 hours.
- Lithium (bipolar disorder): Long-term lithium use can cause elevated blood calcium, and combining it with calcium supplements may increase that risk. Medical supervision is essential.
- Quinolone antibiotics (ciprofloxacin, gemifloxacin, moxifloxacin): Calcium reduces absorption of these antibiotics. Take the antibiotic 2 hours before or 2 hours after calcium supplements.
