Calcium Across a Woman's Life for Long-Term Bone Health

Calcium is the most abundant mineral in the human body, and getting enough of it across every decade of life matters for bone strength, muscle function, and more. This guide walks through what the NIH Office of Dietary Supplements recommends for women at each life stage and explains what the evidence actually supports.
Why Calcium Matters
According to the NIH Office of Dietary Supplements, calcium makes up much of the structure of bones and teeth, keeping tissue rigid, strong, and flexible. Beyond the skeleton, calcium also mediates blood vessel contraction and dilation and supports normal muscle function. These are physiological roles, not disease claims. Supplements are not intended to diagnose, treat, cure, or prevent any disease, and these statements have not been evaluated by the FDA.
How Much Calcium Do Women Need at Each Stage?
The following targets come directly from the NIH Office of Dietary Supplements and reflect Recommended Dietary Allowances established through the Dietary Reference Intakes process. These are population reference values. Talk with a clinician about what is right for your individual situation.
Adolescence and Young Adulthood (Ages 9 to 18)
The teenage years are a critical window for building peak bone mass. The NIH sets the target at 1,300 mg per day for this age group. Reaching that target through dairy, fortified foods, leafy greens, and other calcium-rich sources during these years lays a foundation that carries forward into later life.
Adult Women (Ages 19 to 50)
From age 19 through 50, the NIH target for women is 1,000 mg per day. During pregnancy and lactation, the recommendation stays at 1,000 mg per day for women 19 and older, because the body adjusts calcium absorption efficiency during those periods. Consistently meeting this target through a varied diet supports the physiological roles calcium plays in bone tissue, muscle, and blood vessel function.
Women Over 50
After age 50, the NIH raises the target for women to 1,200 mg per day. Estrogen decline following menopause affects how the body retains calcium, which is why the recommendation increases at this stage. Meeting the higher target becomes especially relevant for long-term skeletal health. The Tolerable Upper Intake Level, which is the maximum daily amount considered safe for most healthy adults, drops to 2,000 mg per day for adults 51 and older, down from 2,500 mg per day for adults ages 19 to 50. Staying below that ceiling from all sources combined, food plus supplements, is important.
Food Sources Versus Supplements
The NIH notes that for most healthy people, calcium from food is safe and well tolerated. Dairy products, calcium-set tofu, canned fish with bones, fortified plant milks, and dark leafy greens such as kale and bok choy are all useful sources. When diet alone does not close the gap, supplements can help.
If you do use a supplement, absorption is best at single doses of 500 mg or less, and taking supplements with food improves absorption. Splitting a larger daily supplement dose into two smaller amounts taken with meals is a practical approach. Calcium carbonate contains more elemental calcium per tablet but requires stomach acid for absorption, making it better suited to taking with food. Calcium citrate is absorbed somewhat more readily and can be taken with or without food, which may be useful for people with lower stomach acid.
What the Evidence Shows
It is worth being clear about what research supports and where evidence is limited, because not all claims about calcium carry the same weight.
- Strong evidence: Calcium is required for normal bone and tooth structure, muscle function, nerve transmission, blood clotting, and blood vessel function.
- Moderate evidence: Supplemental calcium, alone or combined with vitamin D, increases bone mineral density in older adults.
- Limited evidence: Calcium plus vitamin D supplementation reduces the risk of fractures in older adults. The evidence exists but is not conclusive enough to make a firm claim.
- Insufficient evidence: Higher calcium intake lowers the risk of colorectal or other cancers. The data do not currently support this conclusion.
- Limited evidence of risk: Calcium supplements may increase the risk of cardiovascular disease or kidney stones in some people. This is one reason staying below the Tolerable Upper Intake Level matters and why personal medical history should guide supplementation decisions.
Safety and Upper Limits
The NIH Tolerable Upper Intake Level is 2,500 mg per day for adults ages 19 to 50 and 2,000 mg per day for adults 51 and older, counting all sources combined. Exceeding these levels over time is associated with a higher risk of kidney stones and possibly cardiovascular concerns. There is no benefit to taking more calcium than your body can use, and more is not better beyond established targets.
Drug Interactions to Know
Calcium can interact with several common medications. Always talk to a doctor or pharmacist before combining calcium supplements with any prescription drug.
- Dolutegravir (HIV integrase inhibitor): Calcium supplements can substantially reduce blood levels of this medication through chelation. Take dolutegravir 2 hours before or 6 hours after calcium.
- Levothyroxine (thyroid hormone): Calcium carbonate can interfere with absorption of this medication. Separate doses by at least 4 hours.
- Lithium (bipolar disorder): Long-term lithium use can cause elevated calcium levels in the blood, and combining it with calcium supplements may increase that risk.
- Quinolone antibiotics (ciprofloxacin, gemifloxacin, moxifloxacin): Calcium reduces absorption of these antibiotics. Take the antibiotic 2 hours before or 2 hours after calcium supplements.
Putting It Together
Calcium needs are not fixed across a lifetime. They shift with age, hormonal changes, and individual health circumstances. The NIH Office of Dietary Supplements provides clear population targets, but translating those into a personal plan is best done with a clinician or registered dietitian who can account for your diet, medications, and health history. Food-first strategies work well for many women, and supplements can fill genuine gaps when used thoughtfully and within safe limits.
