Do Daily Multivitamins Actually Do Anything for Healthy Adults?
A plain-language look at what the evidence actually says about multivitamin and multimineral supplements, graded honestly from strong to insufficient.

Short answer: For most healthy adults eating a varied diet, a basic daily multivitamin and multimineral supplement (MVM) is unlikely to cause harm, and it reliably increases total nutrient intake. Whether that extra intake translates into meaningful health outcomes depends heavily on who is taking it and why.
What Is a Multivitamin and Multimineral Supplement?
An MVM is a product that combines several vitamins and minerals in a single dose. According to the NIH Office of Dietary Supplements (ODS), there is no standard definition of an MVM, and products vary widely in the number of nutrients included and the amounts provided. Some deliver nutrients close to recommended daily amounts; others are marketed as high-potency formulas that provide doses well above those levels. That distinction matters for both benefit and safety, as explained below.

What the Evidence Actually Shows
Filling Nutritional Gaps (Evidence: Strong)
The most well-supported role for an MVM is straightforward: taking one increases total vitamin and mineral intake and can help people reach recommended amounts when diet alone falls short. The ODS rates this finding as supported by strong evidence. This is not a trivial benefit. National dietary surveys consistently find that a meaningful share of adults do not meet recommended intakes for nutrients such as vitamin D, calcium, potassium, and magnesium from food alone. An MVM can serve as a practical buffer, though the Food and Nutrition Board notes that supplements may not fully compensate for all the benefits of a nutrient-rich diet.
Pregnancy and Early Life (Evidence: Strong to Moderate)
The evidence is strongest for specific life stages. Periconceptional folic acid, commonly obtained through a prenatal MVM, is associated with a reduced risk of neural tube defects, a finding the ODS grades as strong. The Dietary Guidelines for Americans also highlight folic acid, iron, iodine, and vitamin D as key nutrients that prenatal MVMs can reliably supply. Separately, research comparing prenatal MVMs to iron and folic acid alone suggests prenatal MVMs may improve birth outcomes such as low birth weight and preterm birth, though the ODS grades this evidence as moderate rather than strong. Anyone who is pregnant or planning to become pregnant should speak with a clinician about which prenatal supplement is appropriate for their individual situation.
Eye Health in Specific Circumstances (Evidence: Moderate)
A high-dose antioxidant plus zinc formula known as AREDS (Age-Related Eye Disease Study formula) has moderate evidence supporting its use to slow progression from intermediate to advanced age-related macular degeneration in people already diagnosed with that condition. This is a very specific, clinician-guided use case and is distinct from a standard daily MVM. If you or a family member has been diagnosed with macular degeneration, an eye specialist is the right person to advise on whether this formula is appropriate.
Cancer Prevention (Evidence: Limited)
Some observational studies have suggested associations between MVM use and cancer risk, but the ODS grades the overall evidence that MVMs prevent cancer as limited. The U.S. Preventive Services Task Force has also reviewed this question and found insufficient evidence to recommend vitamin and mineral supplements for cancer prevention in the general population. Limited evidence means the research exists but is not consistent or strong enough to draw firm conclusions.
Heart Disease and Mortality (Evidence: Insufficient)
Two of the most common reasons people reach for an MVM are heart health and longevity. The ODS is direct on both counts: evidence that MVMs prevent cardiovascular disease or reduce CVD mortality is insufficient, and evidence that MVMs reduce all-cause mortality is also insufficient. Insufficient does not mean the answer is definitively no, but it does mean the current body of research cannot support a confident yes. The U.S. Preventive Services Task Force similarly found insufficient evidence to recommend supplementation for CVD prevention.
Cognition in Older Adults (Evidence: Limited)
Interest in MVMs for brain health has grown, and some recent trials have reported encouraging signals. The ODS currently grades evidence that MVMs help maintain or enhance cognition in older adults as limited, meaning early findings exist but more rigorous research is needed before strong conclusions can be drawn. If cognitive health is a concern, a clinician can help put any supplement decision in the context of overall lifestyle, diet, and medical history.
Safety: When Does a Multivitamin Become a Risk?
Taking a basic MVM that provides nutrients at approximately recommended amounts is considered safe for healthy people, according to the ODS. Studies of MVM users have not revealed adverse effects beyond minor gastrointestinal complaints, and those complaints occurred at similar rates in placebo groups as well.
The primary safety concern is cumulative excess. If you take an MVM alongside other individual supplements and regularly eat fortified foods such as breakfast cereals, energy bars, or fortified plant milks, your total intake of certain nutrients can climb well above safe upper limits for those nutrients. High-potency MVMs carry the same risk on their own. Because no adult upper limit has been established for MVMs as a category, the risk is tracked nutrient by nutrient. A pharmacist or registered dietitian can help you audit your total daily intake across all sources.
Drug Interactions to Know
The ODS notes that MVMs providing nutrients at recommended amounts do not ordinarily interact with medications, with one important exception. Vitamin K, present in many MVMs, can reduce the effectiveness of warfarin (sold as Coumadin or Jantoven) and similar anticoagulant medications. Anyone taking a blood thinner should talk with their doctor or pharmacist before starting any MVM or supplement that contains vitamin K.
A second interaction worth knowing: proton pump inhibitors and other acid-reducing medications can decrease absorption of food-bound vitamin B12. If you rely on an MVM as your primary source of B12 and also take one of these medications regularly, discuss this with your clinician.
Who Is Most Likely to Benefit?
- People who are pregnant or trying to conceive, particularly for folic acid, iron, iodine, and vitamin D
- People with restricted diets, food allergies, or limited access to a varied diet
- Older adults who may absorb certain nutrients less efficiently
- People with diagnosed deficiencies confirmed by a blood test

A basic MVM is a low-risk way to fill nutrient gaps when diet falls short, and the evidence for that specific role is strong. For broader claims, including heart disease prevention, cancer prevention, and longevity, the evidence ranges from limited to insufficient. The supplement that works best for you depends on your diet, life stage, medications, and health history.
