Fat-Soluble Vitamin Deficiency in Fat Malabsorption and After Bariatric Surgery
People who cannot absorb dietary fat normally face a heightened risk of deficiency in vitamins A, D, and K, and targeted monitoring and supplementation are often necessary.

When the gut cannot absorb dietary fat efficiently, the fat-soluble vitamins A, D, and K travel through the digestive tract without being taken up properly. People who have undergone bariatric surgery or who live with conditions such as Crohn's disease, celiac disease, exocrine pancreatic insufficiency, or short bowel syndrome are among those most likely to be affected.
Why Fat Matters for These Vitamins
Vitamins A, D, and K are absorbed alongside dietary fat in the small intestine. They require bile salts to form micelles, the tiny droplets that carry fat-soluble compounds across the intestinal wall. Any condition that reduces bile flow, damages the intestinal lining, or shortens the absorptive surface can lower the amount of these vitamins that reach the bloodstream. Bariatric procedures such as Roux-en-Y gastric bypass and biliopancreatic diversion with duodenal switch are particularly likely to cause deficiency because they reroute food away from the duodenum and proximal jejunum, the sites where most fat-soluble vitamin absorption occurs.
Vitamin A
Recommended Intakes and Upper Limit
According to the NIH Office of Dietary Supplements, the Recommended Dietary Allowance for vitamin A is 900 micrograms of retinol activity equivalents (RAE) per day for adult men and 700 mcg RAE per day for adult women. When the source is preformed vitamin A (retinol or retinyl esters), 900 mcg RAE equals approximately 3,000 IU. The Tolerable Upper Intake Level for preformed vitamin A is 3,000 mcg per day for adults 19 and older, which is roughly 10,000 IU. No upper limit has been set for provitamin A carotenoids such as beta-carotene from food or supplements.
Physiological Roles and Evidence
Vitamin A is an essential component of rhodopsin, the light-sensitive protein in the retina that supports normal vision in low-light conditions. It also maintains the conjunctival membranes and cornea, supports immune function, and is involved in cellular communication. The evidence that vitamin A is required for normal vision and immune function is graded as strong by the NIH Office of Dietary Supplements.
Safety and Interactions
Preformed vitamin A is stored in the liver and can accumulate to toxic levels. People who are pregnant or who may become pregnant are advised not to take high-dose preformed vitamin A supplements above the UL, given the risk of teratogenic effects. The weight-loss drug orlistat (Alli, Xenical) reduces absorption of fat-soluble vitamins including vitamin A, and manufacturers recommend taking a multivitamin containing vitamin A alongside it. Synthetic retinoid medications such as isotretinoin and acitretin can interact with vitamin A supplements and increase the risk of toxicity. Always talk to a doctor or pharmacist before combining vitamin A supplements with any medication.
Vitamin D
Recommended Intakes and Upper Limit
The NIH Office of Dietary Supplements sets the RDA for vitamin D at 15 mcg (600 IU) per day for adults up to age 70, and 20 mcg (800 IU) per day for adults 71 and older. The Tolerable Upper Intake Level from all sources, including food, beverages, and supplements, is 100 mcg (4,000 IU) per day for everyone age 9 and older, including during pregnancy and lactation. A clinician may prescribe higher doses in specific clinical situations, but self-directed supplementation above the UL is not recommended without medical supervision.
Physiological Roles and Evidence
Vitamin D promotes calcium absorption in the gut and helps maintain serum calcium and phosphate at levels that support normal bone mineralization. Deficiency causes rickets in children and osteomalacia in adults, and this relationship is graded as strong. Evidence that vitamin D combined with calcium supplements modestly increases bone mineral density in some postmenopausal women and older men is graded as moderate. Evidence for vitamin D supplements reducing falls in community-dwelling older adults is graded as limited. Evidence that vitamin D supplements prevent or treat cancer, cardiovascular disease, or type 2 diabetes is graded as insufficient.
Safety and Interactions
Staying within the upper limit avoids the risk of hypercalcemia, a potentially serious condition. Orlistat can reduce absorption of vitamin D from food and supplements. Corticosteroids such as prednisone can lower blood levels of vitamin D. Thiazide diuretics combined with vitamin D supplements may raise blood calcium too high. Consult a clinician or pharmacist before combining vitamin D supplements with any of these medications.
Vitamin K
Recommended Intakes and Upper Limit
Because evidence was insufficient to establish an RDA, the NIH Office of Dietary Supplements sets vitamin K at an Adequate Intake of 120 mcg per day for adult men and 90 mcg per day for adult women. No Tolerable Upper Intake Level has been established for vitamin K.
Physiological Roles and Evidence
Vitamin K acts as a coenzyme for the enzyme required to synthesize clotting proteins, including prothrombin (clotting factor II), which is directly involved in hemostasis. It also supports carboxylation of osteocalcin, a bone protein involved in bone metabolism. Both of these roles are supported by strong evidence. Evidence that vitamin K supplementation reduces the risk of osteoporosis or fracture is graded as limited. Evidence linking higher menaquinone (vitamin K2) intake to lower coronary heart disease risk is also graded as limited.
Safety and Interactions
Vitamin K has low potential for toxicity. However, its interaction with warfarin (Coumadin) and similar anticoagulants is clinically significant. These drugs work by antagonizing vitamin K activity, and sudden changes in vitamin K intake can increase or decrease their anticoagulant effect. People taking warfarin must keep their vitamin K intake consistent and should never start a vitamin K supplement without medical guidance. Orlistat can also reduce vitamin K absorption, and when combined with warfarin it may significantly increase prothrombin time. Bile acid sequestrants such as cholestyramine and colestipol can reduce absorption of vitamin K and other fat-soluble vitamins with long-term use.
Monitoring and Practical Considerations After Bariatric Surgery
Clinical guidelines for bariatric surgery patients typically recommend routine laboratory monitoring of fat-soluble vitamin status before surgery and at regular intervals afterward. The frequency and specific tests vary by procedure type and individual risk factors. A registered dietitian with bariatric experience can help assess dietary intake, and a clinician can order the appropriate blood tests to determine whether supplementation is needed and at what dose.
