Vitamin D, iron, and fluoride doses for infants and toddlers

Three nutrients come up most often when parents and pediatricians talk about early childhood nutrition: vitamin D, iron, and fluoride. This guide summarizes what the NIH Office of Dietary Supplements reports about their roles, reference intakes, and safety limits, so you can have a more informed conversation with your child's clinician.

Vitamin D
What it does
According to the NIH Office of Dietary Supplements, vitamin D promotes calcium absorption in the gut and helps maintain the serum calcium and phosphate levels needed for normal bone mineralization. It also supports bone growth and remodeling by osteoblasts and osteoclasts. Evidence that vitamin D is essential for these skeletal functions is graded strong by the NIH.
Reference intakes for young children
The NIH Office of Dietary Supplements reports the following Dietary Reference Intake values for vitamin D:
- Infants 0 to 12 months: Adequate Intake of 10 mcg (400 IU) per day
- Children 1 to 13 years: Recommended Dietary Allowance of 15 mcg (600 IU) per day
- Tolerable Upper Intake Level for infants 0 to 6 months: 25 mcg (1,000 IU) per day
- Tolerable Upper Intake Level for infants 7 to 12 months: 37.5 mcg (1,500 IU) per day
- Tolerable Upper Intake Level for children 1 to 3 years: 63 mcg (2,500 IU) per day
- Tolerable Upper Intake Level for children 4 to 8 years: 75 mcg (3,000 IU) per day
Breast milk alone typically provides very little vitamin D, which is why many pediatricians recommend a daily supplement for breastfed infants. Formula is fortified, so formula-fed infants who receive the full recommended volume generally meet their needs through formula alone. Your child's clinician is the right person to assess whether a supplement is needed.
Safety and interactions
Staying within the Tolerable Upper Intake Level matters because excess vitamin D can cause hypercalcemia, an abnormally high blood calcium level. The NIH notes that thiazide diuretics combined with vitamin D supplements can raise blood calcium too high, and corticosteroids such as prednisone can lower blood levels of vitamin D. Talk to a doctor before combining vitamin D supplements with any medication.
Iron
What it does
The NIH Office of Dietary Supplements describes iron as an essential component of hemoglobin, the red blood cell protein that carries oxygen from the lungs to the tissues. Iron also forms part of myoglobin, which supplies oxygen to muscle, and it supports physical growth and neurological development. The NIH grades evidence for these physiological roles as strong.

The NIH Office of Dietary Supplements reports the following values:
- Infants 0 to 6 months: Adequate Intake of 0.27 mg per day
- Infants 7 to 12 months: RDA of 11 mg per day
- Children 1 to 3 years: RDA of 7 mg per day
- Children 4 to 8 years: RDA of 10 mg per day
- Tolerable Upper Intake Level for infants 0 to 13 years: 40 mg per day
Full-term breastfed infants are generally born with sufficient iron stores for roughly the first four to six months, but needs rise sharply after that point. Iron-fortified infant formula and iron-rich complementary foods such as pureed meats and fortified cereals are the primary dietary sources for older infants and toddlers. Premature infants may have different requirements, so consult your pediatrician for guidance specific to your child.
Safety and interactions
The NIH notes that the Tolerable Upper Intake Level for children up to age 13 is 40 mg per day, based on gastrointestinal effects. Iron supplements should be stored safely out of reach because accidental ingestion of large amounts is a leading cause of poisoning in young children. The NIH also reports that calcium may reduce the absorption of both nonheme and heme iron, and some experts suggest taking calcium and iron supplements at different times. Speak with a clinician or pharmacist before combining iron with any other supplement or medication.
Fluoride
What it does
According to the NIH Office of Dietary Supplements, fluoride inhibits or reverses the early stages of dental caries (tooth decay) by reducing demineralization and limiting the activity of bacteria in dental plaque. It also enhances remineralization of tooth enamel and is stored in bones and teeth. Evidence that fluoride reduces dental caries in children is graded strong by the NIH. Evidence that fluoride supplements reduce dental caries in children who lack access to fluoridated water is graded moderate.

The NIH Office of Dietary Supplements reports Adequate Intake values for fluoride, noting these are set as AIs rather than RDAs because evidence was not sufficient to establish a full RDA:
- Infants 0 to 6 months: AI of 0.01 mg per day
- Infants 7 to 12 months: AI of 0.5 mg per day
- Children 1 to 3 years: AI of 0.7 mg per day
- Children 4 to 8 years: AI of 1 mg per day
- Tolerable Upper Intake Level for children 1 to 3 years: 1.3 mg per day
- Tolerable Upper Intake Level for children 4 to 8 years: 2.2 mg per day
Most children in the United States who drink fluoridated tap water and use fluoride toothpaste meet their fluoride needs without a separate supplement. Whether a supplement is appropriate depends on the fluoride concentration of your local water supply. Your child's dentist or pediatrician can check local water data and advise accordingly.
Safety
Exceeding the Tolerable Upper Intake Level over time raises the risk of dental fluorosis, a cosmetic change in tooth enamel that appears as faint white streaks in mild cases. The NIH notes that the Tolerable Upper Intake Level for adults is 10 mg per day based on levels associated with dental and skeletal fluorosis. The much lower limits for infants and young children reflect their smaller body size and still-developing teeth.
