The evidence for these fundamental physiological roles is rated strong by the ODS.

Beyond those core functions, the ODS notes that magnesium supports normal muscle and nerve function, contributes to blood glucose control and blood pressure regulation, and plays a role in bone structure. The body works hard to keep serum magnesium stable, drawing from bone and tissue reserves when dietary intake falls short. As a result, serum levels can appear normal even when total body stores are significantly depleted. Clinicians sometimes use red blood cell magnesium testing or a magnesium loading test for a more sensitive assessment, but these are not part of routine panels. Who Is at Greater Risk of Deficiency?

  • Antibiotics: Magnesium can form insoluble complexes with tetracyclines (demeclocycline, doxycycline) and quinolones (ciprofloxacin, levofloxacin), reducing antibiotic absorption. Take these antibiotics at least 2 hours before or 4 to 6 hours after a magnesium-containing supplement.

Supplement Safety and the Upper Limit

Magnesium from food is safe for healthy people because the kidneys excrete any excess efficiently. Supplemental magnesium is a different matter. The NIH ODS sets a Tolerable Upper Intake Level (UL) of 350 mg per day from dietary supplements and medications for anyone age 9 and older. This limit is set specifically to avoid diarrhea and other gastrointestinal effects, which are the most common side effects of excess supplemental magnesium. It does not apply to magnesium naturally present in food and beverages.

Some research, including studies on migraine prevention where the ODS rates the evidence as moderate, has used doses that exceed this UL. Those protocols should only be followed under the direct supervision of a healthcare provider. Do not self-prescribe high-dose magnesium.
