Natural vs Synthetic Vitamin E: d-alpha vs dl-alpha Tocopherol Potency Explained
Understanding the difference between natural and synthetic vitamin E helps you read supplement labels more accurately and make informed choices with your healthcare provider.

Natural and synthetic vitamin E are not identical in potency, and supplement labels reflect this with different IU-to-milligram conversions. Knowing what those labels mean helps you compare products accurately and have a more informed conversation with your doctor or pharmacist.
What Is Vitamin E?
Vitamin E is a fat-soluble nutrient that the body cannot make on its own, so it must come from food or supplements. According to the NIH Office of Dietary Supplements (ODS), vitamin E acts as a fat-soluble antioxidant that stops the production of reactive oxygen species formed when fat undergoes oxidation, protecting cells from free-radical damage. The ODS also notes that vitamin E contributes to normal immune function and is involved in cell signaling, regulation of gene expression, and other metabolic processes, though the evidence for those additional roles comes primarily from in vitro cell studies.
The adult target intake is 15 mg per day, as established in the Dietary Reference Intakes published by the National Academies of Sciences, Engineering, and Medicine.
Natural vs Synthetic: The Core Difference
The letter prefix on the label is your clearest clue. Natural vitamin E is labeled d-alpha-tocopherol. Synthetic vitamin E is labeled dl-alpha-tocopherol. The difference matters because the two forms are not equally active in the body.
Alpha-tocopherol exists as a molecule that can be arranged in different mirror-image configurations, called stereoisomers. The natural form contains only one stereoisomer (RRR-alpha-tocopherol), which the body preferentially absorbs and retains. The synthetic form is produced from petroleum-derived raw materials and contains a mixture of eight stereoisomers, only one of which matches the natural configuration. Because the body does not retain all eight equally well, the synthetic form delivers less biological activity per milligram than the natural form.
IU Conversions: Where the Numbers Come From
Older supplement labels used International Units (IU), and many products still do. The conversion factors below come from the National Academies DRI report and are reflected in ODS guidance:
- 1 IU of natural d-alpha-tocopherol equals 0.67 mg of alpha-tocopherol
- 1 IU of synthetic dl-alpha-tocopherol equals 0.45 mg of alpha-tocopherol
In practical terms, a 400 IU capsule of natural d-alpha-tocopherol provides roughly 268 mg of alpha-tocopherol, while a 400 IU capsule of synthetic dl-alpha-tocopherol provides roughly 180 mg. The IU numbers look the same on the front of the label, but the milligram content differs by about one third. Newer labels required by the FDA express vitamin E in mg of alpha-tocopherol, which makes direct comparison easier.
Upper Limit and Safety
The ODS states that vitamin E from food is considered safe with no established adverse effects. For supplements, however, the Tolerable Upper Intake Level (UL) for adults is 1,000 mg of alpha-tocopherol per day. The ODS notes that this is equivalent to 1,500 IU per day of the natural form or 1,100 IU per day of the synthetic form, and the UL applies to all forms of supplemental alpha-tocopherol. The UL is based on the risk of hemorrhage. Both natural and synthetic forms are subject to the same 1,000 mg ceiling when expressed in milligrams, so neither form is automatically safer at high doses.

Drug Interactions to Know
Before adding a vitamin E supplement to your routine, review any medications you take with a clinician or pharmacist. The ODS identifies three interaction areas worth knowing:
- Anticoagulant and antiplatelet medications such as warfarin (Coumadin): Vitamin E can inhibit platelet aggregation and antagonize vitamin K-dependent clotting factors, so large doses can increase bleeding risk, especially with low vitamin K intake. The ODS notes that the amount needed for a clinically significant effect is unknown but probably exceeds 400 IU per day.
- Simvastatin (Zocor) combined with niacin: In one study, vitamin E taken alongside other antioxidants (vitamin C, selenium, and beta-carotene) blunted the rise in HDL cholesterol, including the cardioprotective HDL2 fraction, in people receiving the simvastatin-niacin combination.
- Chemotherapy and radiotherapy: Oncologists generally advise against antioxidant supplements during cancer treatment because they might reduce effectiveness by protecting cancer cells from oxidative damage. The ODS notes that evidence is mixed and further research is needed.
What the Evidence Actually Shows
It is important to match expectations to the research. The ODS grades the evidence for vitamin E supplementation as follows:
- Strong evidence: Vitamin E functions as a fat-soluble antioxidant and is required for normal physiology, including immune function. Correcting deficiency in people with fat-malabsorption conditions or rare genetic disorders avoids neurological and other deficiency symptoms.
- Insufficient evidence: Vitamin E supplements preventing cardiovascular disease or reducing its morbidity and mortality. Vitamin E supplements preventing cancer. Vitamin E supplements maintaining cognitive performance or slowing cognitive decline and Alzheimer's disease.
- Limited evidence: Vitamin E supplements reducing the risk of age-related macular degeneration or cataracts.
These evidence grades apply regardless of whether the source is natural d-alpha or synthetic dl-alpha tocopherol. Neither form has been shown to prevent or treat any disease, and the research does not currently support supplementation for healthy adults as a strategy to reduce disease risk.
