How much of each key nutrient you need while breastfeeding

Breastfeeding raises your body's demand for several nutrients that pass directly into milk and support your baby's development. The figures below come from the NIH Office of Dietary Supplements and reflect Dietary Reference Intake values set by the National Academies of Sciences, Engineering, and Medicine.

Choline

How much do you need?

The NIH Office of Dietary Supplements notes that no Recommended Dietary Allowance (RDA) has been established for choline because research is not yet sufficient to set one. Instead, an Adequate Intake (AI) is used. For breastfeeding adults, the AI rises to 550 mg per day, the same level set for adult men, reflecting how much choline is transferred into breast milk. The Tolerable Upper Intake Level (UL) for adults is 3,500 mg per day. Most people eating a varied diet do not approach that ceiling.

What does choline do?

  • It provides methyl groups needed for many steps in metabolism.
  • It is required to synthesize phosphatidylcholine and sphingomyelin, two phospholipids that are vital for cell membrane structure. Evidence for this role is rated strong by the NIH.
  • It is needed to produce acetylcholine, a neurotransmitter involved in memory, mood, and muscle control. Evidence for this role is also rated strong.
  • Adequate choline intake is needed for normal liver function and to prevent fat accumulation in the liver. Evidence is rated strong.

Safety and interactions

Intakes below the UL of 3,500 mg per day are considered safe for adults. Intakes above the UL are associated with fishy body odor, excessive sweating, salivation, vomiting, low blood pressure, and liver toxicity. The NIH Office of Dietary Supplements states that choline is not known to have any clinically relevant interactions with medications.

Food sources

Eggs, beef liver, salmon, chicken, and soybeans are among the richest dietary sources. Because many prenatal vitamins contain little or no choline, it is worth checking your label and discussing any gap with your care provider.

Iodine

How much do you need?

The standard adult RDA for iodine is 150 mcg per day, according to the NIH Office of Dietary Supplements. During lactation, that requirement increases substantially to support the amount secreted into breast milk and to maintain your own thyroid function. The adult UL is 1,100 mcg per day. Staying within that range is important, particularly for people with thyroid conditions.

What does iodine do?

  • It is an essential building block of the thyroid hormones thyroxine (T4) and triiodothyronine (T3). Evidence for this role is rated strong.
  • Those hormones regulate metabolic activity, protein synthesis, and enzymatic activity throughout the body.
  • Iodine is required for proper skeletal and central nervous system development in fetuses and infants. Evidence that severe deficiency causes irreversible developmental harm is rated strong by the NIH.

Safety and interactions

Long-term intakes above the UL increase the risk of adverse thyroid effects, including goiter, elevated TSH, hypothyroidism, and iodine-induced hyperthyroidism. People with autoimmune thyroid disease may react to amounts that are well tolerated by others. The NIH Office of Dietary Supplements identifies several medication interactions to be aware of:

  • Antithyroid medications such as methimazole: high iodine doses can have an additive effect and may cause hypothyroidism.
  • Potassium-sparing diuretics such as spironolactone or amiloride: combining these with potassium iodide can raise the risk of high blood potassium (hyperkalemia).
  • ACE inhibitors such as lisinopril or benazepril: combining these with potassium iodide can also raise the risk of hyperkalemia.

If you take any of these medications, speak with your doctor before using an iodine-containing supplement.

Food sources

Seaweed, cod, dairy products, eggs, and iodized salt are common sources. Many prenatal vitamins include iodine, but amounts vary by brand, so reviewing the label is a practical first step.

Folate (Vitamin B9)

How much do you need?

The standard adult RDA for folate is 400 mcg of Dietary Folate Equivalents (DFE) per day, according to the NIH Office of Dietary Supplements. Needs remain elevated during lactation. The UL of 1,000 mcg per day applies specifically to synthetic folic acid from supplements and fortified foods. Folate found naturally in whole foods has no established upper limit and is not associated with harm.

What does folate do?

  • It acts as a coenzyme in single-carbon transfers needed for DNA and RNA synthesis and normal cell division. Evidence is rated strong.
  • It supports amino acid metabolism, including the conversion of homocysteine to methionine, which is involved in producing an important methyl donor called S-adenosyl-methionine.
  • Adequate folic acid taken before conception and in early pregnancy is associated with a reduced risk of neural tube defects. Evidence is rated strong, though this benefit relates primarily to the periconceptional period rather than established lactation.
  • Folate supplementation, alone or with other B vitamins, has been associated with reduced stroke risk, particularly in populations with low folate status. Evidence is rated moderate by the NIH.

Safety and interactions

A key concern with high synthetic folic acid intake is that it can mask a vitamin B12 deficiency and allow nerve damage to progress undetected. The NIH Office of Dietary Supplements highlights several medication interactions:

  • Methotrexate used for cancer treatment: folate supplements can interfere with the drug's mechanism. Consult your provider before supplementing.
  • Antiepileptic medications such as phenytoin, carbamazepine, and valproate: these can lower folate levels, and folate can in turn lower levels of these drugs in the blood.
  • Sulfasalazine for ulcerative colitis: this drug inhibits intestinal folate absorption and can cause folate deficiency.

Food sources

Dark leafy greens, legumes, asparagus, Brussels sprouts, and fortified cereals and breads are reliable sources of folate.
