Omega-3 Fish Oil and Blood Thinner Interactions
What the evidence actually says about combining omega-3 supplements with anticoagulant or antiplatelet medications.

Omega-3 fish oil supplements are among the most widely used dietary supplements in the United States, and many people who take them are also on blood-thinning medications. The short answer is that clinical trials have generally not found this combination to cause clinically significant bleeding, but periodic monitoring is still advised and you should always tell your doctor or pharmacist about every supplement you take.
What Omega-3 Fatty Acids Are and What They Do
The three main omega-3 fatty acids are alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA). According to the NIH Office of Dietary Supplements (ODS), they serve several well-established physiological roles:
- They are structural components of the phospholipids that form cell membranes.
- DHA is especially concentrated in the retina, brain, and sperm.
- They provide a source of energy for the body.
- They act as precursors to eicosanoids, signaling molecules involved in cardiovascular, pulmonary, immune, and endocrine function.
ALA is considered an essential fatty acid, meaning the body cannot make it and it must be obtained from food. This relationship between ALA and dietary intake is supported by strong evidence, per the NIH ODS.
Intake Reference Values From the NIH
The NIH ODS notes that the Institute of Medicine set an Adequate Intake (AI) for ALA rather than a Recommended Dietary Allowance, because the available data were not sufficient to establish an Estimated Average Requirement. The AI for ALA is 1.6 grams per day for adult men and 1.1 grams per day for adult women ages 19 to 50. No specific intake target has been established for EPA or DHA on their own.
Importantly, the Institute of Medicine did not set a Tolerable Upper Intake Level for any omega-3 fatty acid, meaning a formal safe upper limit has not been defined through that process. However, the FDA specifies that dietary supplement labels should not recommend a daily intake of EPA and DHA combined that exceeds 2 grams from supplements. Prescription omega-3 products used to lower triglycerides are dosed well above this level and must be taken only under medical supervision.
The Interaction With Blood Thinners: What the Evidence Shows
This is the central concern for many supplement users. The NIH ODS identifies anticoagulant and antiplatelet medications as a notable interaction category for omega-3 supplements. Common examples include warfarin (Coumadin), aspirin, and clopidogrel (Plavix).
The theoretical concern is straightforward: omega-3 fatty acids can reduce platelet aggregation and may prolong bleeding time. When combined with medications that already reduce clotting, the worry is that bleeding risk could increase further.
However, the NIH ODS also notes that clinical trials have generally shown that any prolonged bleeding time did not exceed normal limits and did not result in clinically significant bleeding. This means the theoretical risk has not consistently translated into a documented clinical harm in research settings. That said, the NIH still advises periodic monitoring when omega-3 supplements are used alongside these medications.
The practical takeaway is not that the combination is necessarily dangerous, but that it warrants open communication with your healthcare team. Your doctor or pharmacist can review your full medication and supplement list, assess your individual risk factors, and advise whether any monitoring of clotting markers such as INR is appropriate for you.
Other Safety Considerations
According to the NIH ODS, omega-3s from food and supplements are generally regarded as safe. Side effects from supplements tend to be mild and may include an unpleasant fishy taste, bad breath, bad-smelling sweat, headache, heartburn, nausea, stomach discomfort, or diarrhea.
There is one additional interaction worth noting. The NIH ODS indicates that high doses, roughly 900 mg per day of EPA plus 600 mg per day of DHA or more taken for several weeks, may suppress inflammatory immune responses and reduce immune function. This is a separate concern from the blood-thinning interaction and is another reason why staying within typical supplement doses and consulting a clinician matters.
These statements have not been evaluated by the FDA, and omega-3 supplements are not intended to diagnose, treat, cure, or prevent any disease.
What the Research Does and Does Not Support
It is worth being clear about what the science currently shows, graded honestly as the NIH ODS presents it:
- Strong evidence: ALA is an essential fatty acid. Omega-3s are structural components of cell membrane phospholipids, with DHA especially concentrated in the retina, brain, and sperm.
- Moderate evidence: EPA and DHA supplements lower elevated serum triglyceride levels.
- Limited evidence: Omega-3 supplements may reduce the risk of some cardiovascular events in the general population. Omega-3 supplements may relieve some symptoms of rheumatoid arthritis.
- Insufficient evidence: Omega-3 supplements improving infant neurodevelopment, preventing cognitive decline or Alzheimer disease, or slowing age-related macular degeneration.
Grading evidence this way matters because it helps set realistic expectations. Moderate or limited evidence means research is promising but not definitive. Insufficient evidence means we do not yet have enough quality data to draw conclusions either way.
Practical Steps Before Combining Omega-3s With Blood Thinners
- Tell your doctor and pharmacist about every supplement you take, including omega-3 fish oil, before starting or continuing a blood-thinning medication.
- Do not exceed the FDA guidance of 2 grams of combined EPA and DHA per day from dietary supplements without medical supervision.
- Ask your doctor whether periodic monitoring of your clotting function is appropriate given your specific medications and health history.
- If you are prescribed a high-dose omega-3 medication for triglycerides, follow your prescriber's instructions exactly, as these are drug-strength doses.
- Do not adjust your prescription medication doses based on supplement use without speaking to a clinician first.
