Vitamin K, omega-3 fatty acids, and ginkgo biloba can all affect how anticoagulant medications work, so timing and dosage deserve careful attention.

Several common supplements can change the way blood-thinning medications work in your body. If you take warfarin or a similar anticoagulant, talking to your doctor or pharmacist before adding any supplement to your routine is an important safety step.
Why this matters for people on anticoagulants
Blood-thinning medications such as warfarin (Coumadin) work within a narrow therapeutic range. Even modest changes in how your body processes clotting factors can shift that balance, either raising the risk of unwanted bleeding or reducing the medication's protective effect. Some supplements influence clotting directly through their nutritional roles, while others affect platelet behavior or interact with the enzymes that metabolize these drugs.

Vitamin K
What it does
Vitamin K acts as a coenzyme for vitamin K-dependent carboxylase, the enzyme required to synthesize proteins involved in hemostasis (blood clotting). It is necessary for the production of prothrombin (clotting factor II), a vitamin K-dependent plasma protein directly involved in blood clotting. Vitamin K also supports the carboxylation of osteocalcin, a bone protein that may be involved in bone mineralization or turnover. The evidence that vitamin K performs these physiological roles is rated strong by the NIH Office of Dietary Supplements.
How much do adults need?
The NIH Office of Dietary Supplements reports an Adequate Intake (AI) of 120 micrograms per day for adult men and 90 micrograms per day for adult women. No Recommended Dietary Allowance (RDA) has been established because the data were insufficient to set an Estimated Average Requirement. No Tolerable Upper Intake Level has been set, reflecting the nutrient's low potential for toxicity.
The warfarin interaction
The most clinically significant safety consideration for vitamin K is its interaction with warfarin and similar vitamin K antagonists, including phenprocoumon, acenocoumarol, and tioclomarol. These anticoagulants work by antagonizing vitamin K activity and depleting vitamin K-dependent clotting factors. Because the drug and the nutrient directly oppose each other, sudden changes in vitamin K intake can increase or decrease the anticoagulant effect. For this reason, people taking these medications are advised to keep their vitamin K intake from both food and supplements consistent day to day, rather than trying to eliminate it entirely.
Other interactions to know
According to the NIH Office of Dietary Supplements, several other medications can affect vitamin K status:
- Antibiotics can destroy the gut bacteria that produce menaquinones (a form of vitamin K), potentially lowering vitamin K status. Cephalosporins such as cefoperazone may also inhibit vitamin K action directly. Supplementation is generally needed only with prolonged antibiotic use beyond several weeks, combined with poor dietary intake.
- Bile acid sequestrants such as cholestyramine (Questran) and colestipol (Colestid) can reduce absorption of vitamin K and other fat-soluble vitamins. Status should be monitored with long-term use.
- Orlistat (Alli, Xenical) reduces dietary fat absorption and can lower absorption of fat-soluble vitamins including vitamin K. When combined with warfarin, orlistat may significantly increase prothrombin time.
Omega-3 fatty acids (EPA and DHA)
What they do
Omega-3 fatty acids serve as structural components of the phospholipids that form cell membranes, with DHA especially concentrated in the retina, brain, and sperm. They also act as precursors to eicosanoids, signaling molecules with wide-ranging roles in the cardiovascular, pulmonary, immune, and endocrine systems. The evidence for these structural and metabolic roles is rated strong.
How much do adults need?
The NIH Office of Dietary Supplements reports an AI for alpha-linolenic acid (ALA), the essential plant-based omega-3, of 1.6 grams per day for adult men and 1.1 grams per day for adult women ages 19 to 50. No RDA or specific intake recommendation has been established for EPA or DHA individually. No Tolerable Upper Intake Level has been set for any omega-3. The FDA specifies that dietary supplement labels should not recommend a daily intake of EPA and DHA higher than 2 grams from supplements.
The anticoagulant interaction
Because omega-3s can reduce platelet aggregation and prolong bleeding time, combining them with anticoagulant or antiplatelet medications such as warfarin, aspirin, or clopidogrel may theoretically increase bleeding risk. Clinical trials have generally shown that the prolongation of bleeding time did not exceed normal limits or cause clinically significant bleeding, but the NIH Office of Dietary Supplements advises periodic monitoring when these are used together. High doses, roughly 900 mg per day of EPA plus 600 mg per day of DHA or more for several weeks, may also suppress inflammatory immune responses. Prescription high-dose omega-3 products used to lower triglycerides should be taken only under medical supervision.
Ginkgo biloba
What it does
Ginkgo leaf extract contains flavonoids and terpene lactones and is widely marketed for cognitive support and circulation. However, the National Center for Complementary and Integrative Health (NCCIH) notes that research has not established a benefit for memory or thinking in healthy people. Evidence that ginkgo relieves symptoms of existing dementia is rated limited, and evidence for preventing dementia or cognitive decline is rated insufficient.
The bleeding and anticoagulant interaction
Ginkgo may increase the risk of bleeding, and this risk may be compounded in people taking anticoagulant or antiplatelet drugs such as warfarin. The NCCIH advises consulting a health care provider before combining ginkgo with any medication. Ginkgo may also be unsafe during pregnancy because it might cause early labor or extra bleeding during delivery.
