Can You Take Calcium and Iron Together or Space Them Out
Evidence from the NIH Office of Dietary Supplements suggests calcium may reduce iron absorption, so timing your supplements thoughtfully can help you get the most from both.
The short answer is that taking calcium and iron at the same time may reduce how well your body absorbs iron, so spacing them apart by a few hours is a reasonable precaution. The evidence behind this interaction is not yet definitive, but the practical cost of separating the doses is low and the potential benefit is real.
Why These Two Nutrients Matter
Before getting into timing, it helps to understand what each nutrient does in the body, according to the NIH Office of Dietary Supplements (ODS).
What Calcium Does
- Makes up much of the structure of bones and teeth, keeping tissue rigid, strong, and flexible.
- Mediates blood vessel contraction and dilation.
- Supports normal muscle function and nerve transmission.
What Iron Does
- Serves as an essential component of hemoglobin, the red blood cell protein that carries oxygen from the lungs to the tissues.

Both nutrients play foundational physiological roles. The NIH ODS rates the evidence that iron is essential for hemoglobin formation and oxygen transport as Strong, and the evidence that calcium is required for normal bone and tooth structure, muscle function, nerve transmission, and blood vessel function as equally Strong.
The Calcium and Iron Interaction
According to the NIH ODS, calcium may reduce the bioavailability of both nonheme iron (the form found in plant foods and most supplements) and heme iron (the form found in meat). The agency notes, however, that this effect is not definitively established. Some experts suggest taking calcium and iron supplements at different times of day as a practical way to sidestep the potential interference.
In plain terms: the interaction is plausible and biologically consistent, but the research has not yet produced a firm consensus on exactly how large the effect is in everyday conditions. Grading this honestly, the evidence sits closer to Limited than Strong. That uncertainty is not a reason to ignore the issue entirely, especially for people who already have low iron stores or who are pregnant.
Daily Targets and Upper Limits
All figures below come from the NIH ODS and the National Academies Dietary Reference Intakes.
Calcium
- Adults 19 to 50: 1,000 mg per day.
- Women 51 and older: 1,200 mg per day.
- Men 71 and older: 1,200 mg per day.
- Tolerable Upper Intake Level: 2,500 mg per day for adults 19 to 50; 2,000 mg per day for adults 51 and older, from all sources combined.
Iron
- Men 19 to 50: 8 mg per day.
- Women 19 to 50: 18 mg per day.
- Tolerable Upper Intake Level: 45 mg per day for adults, based on gastrointestinal effects.
Higher iron doses are sometimes prescribed by clinicians to replenish stores in people with documented deficiency, but that decision belongs with a healthcare provider. Please consult a clinician or pharmacist for personal dosing guidance rather than adjusting amounts on your own.
Practical Timing Suggestions
If you take both supplements, the simplest approach is to separate them by at least a few hours. A common pattern is to take iron in the morning, ideally with a small amount of vitamin C to support absorption, and to take calcium later in the day with a meal.
On the calcium side, the NIH ODS notes that absorption is best at single doses of 500 mg or less, and that taking supplements with food improves absorption. Splitting a larger calcium dose across two meals rather than taking it all at once is consistent with this guidance.
On the iron side, taking the supplement away from high-calcium foods and beverages, such as dairy or fortified plant milks, follows the same logic as separating the supplements themselves.
Other Interactions to Know About
Both nutrients interact with certain prescription medications. Talk to a doctor before combining either supplement with any medication.
Calcium Interactions
- Dolutegravir (HIV integrase inhibitor): calcium can substantially reduce blood levels of this drug through chelation. The NIH ODS advises taking dolutegravir 2 hours before or 6 hours after calcium supplements.
- Levothyroxine (thyroid hormone): calcium carbonate can interfere with absorption. Separate doses by at least 4 hours.
- Quinolone antibiotics such as ciprofloxacin, gemifloxacin, and moxifloxacin: calcium reduces antibiotic absorption. Take the antibiotic 2 hours before or 2 hours after calcium supplements.
- Lithium: long-term use can cause elevated blood calcium, and combining with calcium supplements may increase that risk.
Iron Interactions
- Levothyroxine: taking iron at the same time as levothyroxine can reduce the thyroid medication's absorption and efficacy. Labels advise separating iron by at least 4 hours.
- Levodopa (in Sinemet and Stalevo): iron supplements can reduce levodopa absorption, possibly by chelation, and may reduce its clinical effectiveness.
- Proton pump inhibitors such as lansoprazole and omeprazole: these drugs lower stomach acidity and can reduce nonheme iron absorption.
Safety Considerations
For most healthy adults, calcium from food is safe. Staying below the Tolerable Upper Intake Level matters most for supplement users. The NIH ODS notes that higher supplemental intakes may increase the risk of kidney stones and possibly cardiovascular disease, though the evidence on cardiovascular risk is rated as Limited.
For iron, adults with normal intestinal function have very little risk of iron overload from food. The 45 mg per day upper limit applies to combined food and supplement intake. Exceeding it without clinical supervision is not advisable.
