Vitamin D and Calcium Needs During Pregnancy
A plain-language look at what the NIH recommends for two nutrients that support bone health and more during pregnancy.

Vitamin D and calcium work closely together to support bone development, muscle function, and more during pregnancy. Here is what current NIH Office of Dietary Supplements (ODS) data says about daily targets, safe upper limits, and a few things worth discussing with your care team.
Why These Two Nutrients Matter
Calcium makes up much of the structure of bones and teeth, keeping tissue rigid, strong, and flexible. It also mediates blood vessel contraction and dilation and supports normal muscle function. Vitamin D promotes calcium absorption in the gut and helps maintain adequate serum calcium and phosphate concentrations so that normal bone mineralization can occur. Vitamin D also helps prevent hypocalcemic tetany, the involuntary muscle contractions, cramps, and spasms that can result from low blood calcium. Together, these two nutrients play a central role in the skeletal development of a growing baby as well as the bone health of the pregnant person.
Daily Targets During Pregnancy
According to the NIH ODS, the recommended daily amounts do not change specifically for pregnancy compared with other adults in the same age range, but meeting those targets consistently becomes especially important.
- Vitamin D: 15 mcg (600 IU) per day for adults up to age 70.
- Calcium: 1,000 mg per day for adults ages 19 to 50.
These figures are population reference values. Your clinician may recommend a different amount based on your individual health history, blood levels, and diet. Always ask your doctor or midwife before adjusting your intake.
Safe Upper Limits
The Tolerable Upper Intake Level (UL) is the highest daily amount considered safe for most healthy people from all sources combined, including food, beverages, and supplements.
- Vitamin D: 100 mcg (4,000 IU) per day. This UL applies during pregnancy and lactation. Staying within it helps avoid hypercalcemia, a condition of abnormally high blood calcium. A health care provider may prescribe higher doses in specific clinical situations, but that should only happen under medical supervision.
- Calcium: 2,500 mg per day for adults ages 19 to 50. Higher supplemental intakes above this level may increase the risk of kidney stones and possibly cardiovascular disease, based on available evidence.
Food Sources to Know
Getting nutrients from food first is a reasonable approach. Useful dietary sources include the following.
Vitamin D
- Fatty fish such as salmon, trout, and tuna
- Fortified dairy milk and plant-based milk alternatives
- Fortified orange juice and breakfast cereals
- Egg yolks
Calcium
- Dairy products including milk, yogurt, and cheese
- Fortified plant-based milks
- Leafy greens such as kale and bok choy
- Canned fish with soft bones, such as sardines and salmon
- Fortified tofu
If You Take Supplements
Many prenatal vitamins include both vitamin D and calcium. A few practical points from the NIH ODS are worth keeping in mind.
- Calcium absorption is best at single doses of 500 mg or less. If your total supplemental calcium is higher than that, splitting doses across the day may help.
- Taking calcium supplements with food can improve absorption.
- Vitamin D and calcium supplements are not intended to diagnose, treat, cure, or prevent any disease, and these statements have not been evaluated by the FDA.
Drug Interactions to Discuss With Your Doctor or Pharmacist
Both nutrients can interact with certain medications. If you take any of the following, talk to your doctor or pharmacist before adding or adjusting supplements.
Vitamin D interactions
- Orlistat (Xenical, alli): This weight-loss drug can reduce absorption of vitamin D from food and supplements.
- Statins (for example atorvastatin, lovastatin, simvastatin): High-dose vitamin D supplements may reduce how well these medications work.
- Corticosteroids such as prednisone: Can lower blood levels of vitamin D.
- Thiazide diuretics (for example hydrochlorothiazide, chlorthalidone): Combined with vitamin D supplements, these can raise blood calcium too high.
Calcium interactions
- Dolutegravir (HIV integrase inhibitor): Calcium supplements can substantially reduce blood levels of this medication through chelation. Take dolutegravir 2 hours before or 6 hours after calcium.
- Levothyroxine (thyroid hormone): Calcium carbonate can interfere with absorption. Separate doses by at least 4 hours.
- Lithium: Long-term use can cause hypercalcemia, and combining with calcium supplements may increase that risk.
- Quinolone antibiotics (ciprofloxacin, moxifloxacin, gemifloxacin): Calcium reduces antibiotic absorption. Take the antibiotic 2 hours before or 2 hours after calcium supplements.
What the Research Actually Shows
It helps to understand how confident researchers are in various claims. The NIH ODS grades evidence in a way that is worth reflecting here honestly.
- Strong evidence: Calcium is required for normal bone and tooth structure, muscle function, nerve transmission, blood clotting, and blood vessel function. Vitamin D promotes intestinal calcium absorption and maintains serum calcium and phosphate for normal bone mineralization. Vitamin D deficiency causes rickets in children and osteomalacia in adults.
- Moderate evidence: Vitamin D plus calcium supplements slightly increase bone mineral density in some postmenopausal women and older men and can reduce fracture rates in institutionalized older people. Supplemental calcium, alone or with vitamin D, increases bone mineral density in older adults.
- Limited evidence: Calcium plus vitamin D supplementation reduces the risk of fractures in older adults. Vitamin D supplements reduce falls or fractures in community-dwelling older adults. Calcium supplements may increase the risk of cardiovascular disease or kidney stones.
- Insufficient evidence: Current research does not support the claim that vitamin D supplements prevent or treat cancer, cardiovascular disease, depression, multiple sclerosis, or type 2 diabetes, or that they aid weight loss. Evidence is also insufficient to conclude that higher calcium intake lowers the risk of colorectal or other cancers.
The Bottom Line
Meeting the NIH-recommended daily targets for vitamin D (15 mcg / 600 IU) and calcium (1,000 mg) supports the physiological functions these nutrients are known to perform during pregnancy. Staying below the established upper limits helps reduce the risk of adverse effects.
