Vitamin D2 vs D3: Which Raises Blood Levels More Effectively

Short answer: Both vitamin D2 (ergocalciferol) and vitamin D3 (cholecalciferol) raise blood levels of vitamin D, but most research suggests D3 raises and sustains those levels more effectively over time. For most healthy adults, the NIH Office of Dietary Supplements sets the daily target at 15 mcg (600 IU), so the difference between forms is unlikely to matter unless your clinician has identified a deficiency.
Why Vitamin D Form Matters
Vitamin D is not a single compound. The two forms sold as supplements come from different sources and behave somewhat differently once they enter the body. Understanding that difference helps you have a more informed conversation with your doctor or pharmacist.
- Vitamin D2 (ergocalciferol) is derived from plant sources and fungi, including UV-exposed yeast and mushrooms. It has been used in prescription-strength supplements for decades.
- Vitamin D3 (cholecalciferol) is the form the human body makes when skin is exposed to ultraviolet B sunlight. It is also found in fatty fish, fish liver oils, egg yolks, and fortified foods. Most over-the-counter supplements sold in the United States use D3.
Both forms are converted by the liver and kidneys into the active hormone calcitriol, which is the form the body actually uses. The debate is about how efficiently each form gets you there.
What the Research Shows
Several clinical trials have compared D2 and D3 head-to-head by measuring serum 25-hydroxyvitamin D, the standard blood marker used to assess vitamin D status. The consistent finding is that D3 raises and maintains serum levels more effectively than an equivalent dose of D2, particularly when supplements are taken weekly or less frequently rather than daily. When both forms are taken daily, the gap narrows, though D3 still tends to perform better in most studies.
It is important to be honest about the strength of this evidence. The NIH ODS notes that vitamin D's core physiological roles, such as promoting intestinal calcium absorption and maintaining serum calcium and phosphate for normal bone mineralization, are supported by strong evidence. However, much of the head-to-head D2 versus D3 comparison literature involves relatively small samples and varying dosing schedules. The practical clinical significance of the difference, meaning whether choosing D3 over D2 actually leads to better health outcomes, has not been firmly established in large long-term trials.
What Vitamin D Actually Does in the Body
According to the NIH Office of Dietary Supplements, vitamin D performs several well-established physiological functions regardless of which form you take:
- Promotes calcium absorption in the gut and maintains adequate serum calcium and phosphate concentrations to enable normal bone mineralization (evidence grade: strong).
- Prevents hypocalcemic tetany, which involves involuntary muscle contractions, cramps, and spasms.
- Supports bone growth and remodeling by osteoblasts and osteoclasts, and together with calcium helps protect older adults from osteoporosis.
Beyond bone health, some research has examined whether vitamin D supplements might help with cancer, cardiovascular disease, depression, multiple sclerosis, type 2 diabetes, or weight loss. The NIH ODS rates the evidence for all of those outcomes as insufficient, meaning current research does not support using vitamin D supplements specifically to address those conditions.
Vitamin D plus calcium supplements have shown a moderate evidence benefit for slightly increasing bone mineral density in some postmenopausal women and older men, and for reducing fracture rates in institutionalized older adults. Evidence that vitamin D supplements reduce falls or fractures in community-dwelling older adults is rated as limited by the NIH ODS.
Daily Targets and the Upper Limit
The NIH Office of Dietary Supplements provides the following reference values for vitamin D from all sources combined, meaning food, beverages, and supplements together:
- Adults aged 19 to 70: 15 mcg (600 IU) per day
- Adults aged 71 and older: 20 mcg (800 IU) per day
- Tolerable Upper Intake Level for everyone aged 9 and older, including during pregnancy and lactation: 100 mcg (4,000 IU) per day
Staying within the upper limit matters because excessive vitamin D from supplements can cause hypercalcemia, a condition involving abnormally high blood calcium. Symptoms can include nausea, weakness, and kidney problems. A health care provider may prescribe doses above the upper limit in specific clinical situations, but that should only happen under medical supervision with regular blood monitoring.
Drug Interactions to Know
If you take any prescription medications, talk to your doctor or pharmacist before adding a vitamin D supplement. The NIH Office of Dietary Supplements identifies several interactions worth knowing:
- Orlistat (Xenical, alli): This weight-loss drug can reduce absorption of vitamin D from food and supplements.
- Statins such as atorvastatin (Lipitor), lovastatin (Altoprev, Mevacor), and simvastatin (FloLipid, Zocor): High-dose vitamin D supplements may reduce how well statins work.
- Corticosteroids such as prednisone (Deltasone, Rayos, Sterapred): These medications can lower blood levels of vitamin D.
- Thiazide diuretics such as chlorthalidone (Hygroton), indapamide (Lozol), and hydrochlorothiazide (Microzide): When combined with vitamin D supplements, these drugs can raise blood calcium to unsafe levels.
Which Form Should You Choose?
For most people buying an over-the-counter supplement to fill a modest dietary gap, D3 is the more commonly recommended choice based on the available comparative research. If you have been prescribed a high-dose D2 supplement by your doctor, that prescription is appropriate and effective for its intended purpose. Switching forms without telling your clinician is not recommended.
The more important question is whether you need a supplement at all. Blood levels vary considerably depending on sun exposure, diet, skin tone, age, body weight, and underlying health conditions. A simple blood test ordered by your doctor can tell you where you actually stand.
