What vitamin D actually does in the body

It promotes calcium absorption in the gut and helps maintain adequate serum calcium and phosphate concentrations, which are necessary for normal bone mineralization. It also prevents hypocalcemic tetany, the involuntary muscle contractions, cramps, and spasms that occur when blood calcium falls too low. Over a lifetime, vitamin D supports bone growth and remodeling by osteoblasts and osteoclasts, and together with calcium it helps protect older adults from osteoporosis.
The evidence behind these bone-related functions is graded as strong by the ODS. Vitamin D deficiency is the established cause of rickets in children and osteomalacia in adults. The picture is less clear for other health areas: vitamin D plus calcium supplements show moderate evidence for slightly increasing bone mineral density in some postmenopausal women and older men, and for reducing fracture rates in institutionalized older adults. Evidence for reducing falls or fractures in community-dwelling older adults is limited. For cancer, cardiovascular disease, depression, multiple sclerosis, type 2 diabetes, or weight loss, current evidence is insufficient to draw conclusions.
Where D3 and D2 come from
Vitamin D3 (cholecalciferol) is the form your skin synthesizes when exposed to ultraviolet B sunlight. It is also found in fatty fish, egg yolks, and most fortified dairy products. Most over-the-counter supplements use D3, which is typically derived from lanolin in sheep's wool or, in vegan formulations, from lichen.
Vitamin D2 (ergocalciferol) comes from plant sources, primarily UV-irradiated yeast and fungi. It has been used in prescription-strength vitamin D for decades and is the form found in some fortified plant-based milks and cereals. Because it comes from non-animal sources, it is the default choice in vegan supplements.
Does the form matter for absorption and blood levels
Both D3 and D2 are absorbed in the small intestine and then converted by the liver and kidneys into the active hormone form the body uses. Multiple clinical studies suggest that D3 raises and sustains blood 25-hydroxyvitamin D concentrations somewhat more effectively than an equivalent dose of D2, particularly over longer supplementation periods. The practical gap appears modest at typical supplementation doses, but it becomes more relevant when dosing intervals are longer, such as weekly or monthly bolus dosing, where D3 appears to hold its advantage more clearly.
For most people choosing a daily supplement in the standard dose range, either form can be adequate. If you follow a vegan diet and want to avoid animal-derived ingredients, a D2 or lichen-derived D3 product can still support healthy vitamin D status when taken consistently.
How much do you actually need
The NIH ODS provides the following population reference values. The Recommended Dietary Allowance for adults up to age 70 is 15 micrograms (600 IU) per day. For adults aged 71 and older, the target rises to 20 micrograms (800 IU) per day. These figures apply to both D3 and D2 combined from all sources including food, beverages, and supplements.
The Tolerable Upper Intake Level, meaning the maximum daily amount considered safe for most healthy people, is 100 micrograms (4,000 IU) per day for everyone aged 9 and older, including during pregnancy and lactation. Staying within this limit matters because excess vitamin D raises blood calcium to levels that can cause hypercalcemia, a condition with symptoms ranging from nausea and weakness to more serious complications. A healthcare provider may prescribe higher doses in supervised clinical situations, but that is a decision for a clinician, not a supplement label.

Drug interactions you should know about
Vitamin D supplements can interact with several common medications. The ODS highlights the following:

- Statins such as atorvastatin (Lipitor), lovastatin (Altoprev, Mevacor), and simvastatin (FloLipid, Zocor): high-dose vitamin D supplements may reduce how well these cholesterol-lowering medications work.
- Corticosteroids such as prednisone (Deltasone, Rayos, Sterapred): long-term use can lower blood levels of vitamin D.
- Thiazide diuretics such as chlorthalidone (Hygroton), indapamide (Lozol), and hydrochlorothiazide (Microzide): when combined with vitamin D supplements, these medications can push blood calcium too high.
