What Blocks Folate Absorption and How to Avoid It
Several medications, dietary habits, and genetic factors can reduce how much folate your body actually uses, but practical steps can help you stay on track.

Short answer: Folate absorption can be reduced by certain medications, alcohol, heat from cooking, and a common genetic variation that affects how your body converts folate into its active form. Understanding these factors helps you make informed choices about food sources and, when appropriate, supplementation.
Why Folate Matters
Folate is a B vitamin your body cannot make on its own, so it must come from food or supplements. According to the NIH Office of Dietary Supplements (ODS), folate acts as a coenzyme in single-carbon transfers that are essential for DNA and RNA synthesis. It also supports amino acid metabolism, including the conversion of homocysteine to methionine in the production of S-adenosyl-methionine, an important methyl donor. Additionally, folate enables the methylation of deoxyuridylate to thymidylate, a step required for proper cell division.
The NIH ODS sets the adult daily target at 400 micrograms of dietary folate equivalents (mcg DFE). The tolerable upper intake level for synthetic folic acid from supplements and fortified foods is 1,000 mcg per day for adults. Folate found naturally in food and beverages is not harmful and has no established upper limit.
How Folate Is Absorbed
Most dietary folate arrives as polyglutamate forms that must be broken down in the intestinal lining before absorption. Synthetic folic acid, used in supplements and fortified foods, is already in a simpler form and is absorbed more efficiently than food folate. Once inside cells, an enzyme called dihydrofolate reductase converts folic acid into active folate. Anything that interferes with this enzyme or with the intestinal breakdown process can reduce how much folate reaches your tissues.
Key Factors That Block or Reduce Folate Absorption
1. Medications With Known Interactions
The NIH ODS identifies several medications that affect folate levels. Always talk to a doctor or pharmacist before combining any supplement with a prescription medication.
- Methotrexate (Rheumatrex, Trexall): This medication works partly by inhibiting dihydrofolate reductase, the same enzyme that activates folic acid. When methotrexate is used to treat cancer, folate supplements can interfere with that intended effect. Patients on methotrexate for any condition should consult their provider before supplementing with folate.
- Antiepileptic medications such as phenytoin (Dilantin), carbamazepine (Carbatrol, Tegretol, Equetro, Epitol), and valproate (Depacon) can reduce blood levels of folate. In turn, folate supplements can reduce blood levels of these medications. Anyone taking antiseizure drugs should discuss supplementation with their neurologist or prescribing physician.
- Sulfasalazine (Azulfidine), used for ulcerative colitis, directly inhibits intestinal folate absorption and can lead to folate deficiency over time.
2. Alcohol
Regular alcohol consumption interferes with folate absorption in the small intestine and increases urinary excretion of folate. People who drink heavily are at elevated risk of low folate status and may benefit from reviewing their dietary intake with a clinician.
3. Heat and Food Preparation
Folate is water-soluble and sensitive to heat. Boiling vegetables for extended periods can destroy a significant portion of their folate content. Steaming, microwaving with minimal water, or eating folate-rich foods raw when appropriate can help preserve more of the vitamin. Dark leafy greens, legumes, and fortified cereals are among the richest food sources.
4. The MTHFR Genetic Variation
A common genetic variation in the MTHFR gene can reduce the activity of an enzyme that converts folate into its most active circulating form, 5-methyltetrahydrofolate (5-MTHF). People with this variation may have modestly lower folate status even with adequate intake. Some clinicians recommend the 5-MTHF form of folate for these individuals, though personal guidance from a healthcare provider is essential before making that change.
5. Malabsorptive Conditions
Conditions that affect the small intestine, such as celiac disease and inflammatory bowel disease, can impair nutrient absorption broadly, including folate. If you have a diagnosed gastrointestinal condition, ask your doctor whether your folate status has been checked.

- Strong evidence: Folate is required as a coenzyme for DNA and RNA synthesis, amino acid metabolism, and normal cell division. Adequate folic acid before conception and in early pregnancy is associated with a reduced risk of neural tube defects such as anencephaly and spina bifida.
- Moderate evidence: Folate supplementation, alone or combined with other B vitamins, is associated with a reduced risk of stroke, particularly in populations with low baseline folate status.
- Limited evidence: Folate naturally present in food may be associated with a lower risk of some cancers, while high-dose folic acid may have complex and possibly harmful effects in this area. Folate supplements, particularly the 5-MTHF form, have shown some association with improved depression symptoms or antidepressant response in preliminary research.
- Insufficient evidence: Research on folic acid supplementation for cognitive function, dementia prevention, or reducing autism spectrum disorder risk in offspring does not yet support firm conclusions.
