Zinc is an essential mineral that supports hundreds of enzymes, immune defenses, and the repair of tissues throughout the body. Most people in the United States consume adequate amounts, according to the NIH Office of Dietary Supplements, but certain groups face a higher risk of falling short.

What Zinc Does in the Body

According to the NIH Office of Dietary Supplements, zinc is required for the catalytic activity of hundreds of enzymes and plays a central role in immune function, protein and DNA synthesis, wound healing, and cell division. This breadth of function explains why inadequate intake can produce a wide range of physical signs. The evidence that zinc is essential for these physiological processes is rated strong by the NIH.

Recognizing Possible Signs of Deficiency

Zinc deficiency exists on a spectrum. Severe deficiency is rare in high-income countries but mild or marginal deficiency can occur in older adults, people with gastrointestinal conditions that limit absorption, vegetarians, and those with alcohol dependence. Common signs that clinicians associate with inadequate zinc status include the following.

Immune and Infection-Related Signs

  • Frequent or prolonged infections, because zinc is required for normal development and function of immune cells
  • Slow recovery from minor illnesses
  • Impaired wound healing, reflecting zinc's role in cell division and tissue repair

Skin and Hair Signs

  • Dermatitis or skin lesions, particularly around the mouth, hands, and feet
  • Hair thinning or increased hair shedding
  • Delayed healing of cuts and abrasions

Taste, Smell, and Appetite Signs

  • Reduced sense of taste (hypogeusia) or smell, because zinc-dependent enzymes are involved in sensory function
  • Decreased appetite, which can compound the original dietary shortfall

Other Reported Signs

  • Diarrhea in children, particularly in low-income settings where deficiency is more prevalent
  • Growth delays in children and adolescents
  • Difficulty with night vision, linked to zinc's role in vitamin A metabolism in the eye

None of these signs alone confirms a zinc deficiency. Many overlap with other nutrient shortfalls or medical conditions, which is why clinical assessment is necessary before starting supplementation.

How Zinc Status Is Assessed

There is no single reliable biomarker for zinc status. Serum or plasma zinc concentration is the most widely used clinical measure, but it can be influenced by infection, inflammation, and the time of day the sample is taken. A clinician may combine a blood test with a detailed dietary history and a review of physical signs to form an overall picture. If you are concerned about your zinc status, speak with a healthcare provider rather than self-diagnosing based on symptoms alone.

How Much Zinc Do Adults Need

The NIH Office of Dietary Supplements sets the Recommended Dietary Allowance at 11 mg per day for adult men and 8 mg per day for adult women. These figures cover the needs of most healthy adults through food and, where necessary, supplements combined. Good dietary sources include red meat, poultry, shellfish (especially oysters), beans, nuts, whole grains, and fortified cereals.

The Upper Limit and Why It Matters

The Tolerable Upper Intake Level for adults is 40 mg per day from food and supplements combined, as established by the NIH Office of Dietary Supplements and the National Academies of Sciences. This limit is based on levels that adversely affect copper status. Doses of 50 mg or more per day taken over several weeks can reduce copper status and impair immune function, which is the opposite of the intended effect. The upper limit does not apply to individuals receiving zinc for medical treatment under the care of a physician.

What the Research Shows

It is important to understand what the current evidence does and does not support.

  • Common cold duration (Limited evidence): Supplemental zinc in lozenge or syrup form taken shortly after cold onset may reduce the duration, but not the severity, of common cold symptoms. The NIH rates this evidence as limited.
  • Age-related macular degeneration (Moderate evidence): AREDS-type supplements containing zinc plus copper and antioxidants slow the progression of age-related macular degeneration in people at high risk of the advanced form. This finding applies to a specific high-risk population and a specific formulation.
  • Diarrhea in children (Moderate evidence): Zinc supplementation reduces the duration of acute diarrhea in children in low-income countries where deficiency is common.
  • Pneumonia in children (Limited evidence): Zinc supplementation reduces the incidence and prevalence of pneumonia in children in low-income countries where deficiency is common.
  • Type 2 diabetes and HIV (Insufficient evidence): Current evidence is insufficient to conclude that zinc supplementation prevents or improves type 2 diabetes or benefits people with HIV.

Drug and Supplement Interactions

Zinc can interact with several medications and nutrients. Always talk to a doctor or pharmacist before combining zinc supplements with any medication.

  • Quinolone and tetracycline antibiotics (for example, ciprofloxacin): zinc can inhibit absorption of both the antibiotic and the zinc itself. The antibiotic should be taken at least 2 hours before or 4 to 6 hours after the zinc supplement.
  • Penicillamine (used for rheumatoid arthritis and Wilson disease): zinc can reduce its absorption and action. Take zinc and penicillamine at least 1 hour apart.
  • Thiazide diuretics such as chlorthalidone and hydrochlorothiazide increase urinary zinc excretion, which can decrease serum zinc concentrations over time.
  • Iron supplements containing 25 mg or more of elemental iron taken at the same time as zinc can reduce zinc absorption and plasma zinc concentrations.

If you recognize several of the signs described above, or if you belong to a group at higher risk of deficiency, the appropriate next step is a conversation with a healthcare provider. A clinician can order relevant tests, review your diet and medications, and advise on whether supplementation is appropriate for your individual situation. Personal dosing decisions should not be based on general population reference values alone.
