Daily vitamin B6 dose and the nerve safety upper limit

Most healthy adults need just 1.3 mg of vitamin B6 per day, an amount most people reach through food alone. The safety conversation matters because long-term high-dose supplementation can damage sensory nerves, a risk that two major health authorities now quantify very differently.
What vitamin B6 actually does in the body
Vitamin B6 is not a single compound. It is a family of related forms that the body converts into pyridoxal 5-phosphate (PLP), the active coenzyme. According to the NIH Office of Dietary Supplements (ODS), PLP participates in more than 100 enzyme reactions, the majority of which involve protein and amino acid metabolism. It also supports the metabolism of one-carbon units, carbohydrates, and lipids.
Beyond basic metabolism, vitamin B6 contributes to cognitive development through the biosynthesis of neurotransmitters, and it helps maintain normal blood homocysteine levels. The evidence that vitamin B6 performs these physiological roles is graded as strong by the NIH ODS.
The recommended daily intake for adults
The Dietary Reference Intake established by the Food and Nutrition Board (FNB) sets the Recommended Dietary Allowance (RDA) for vitamin B6 at 1.3 mg per day for adults aged 19 to 50, for both men and women. This figure comes from the National Academies of Sciences report on B vitamins and is cited directly by the NIH ODS. Requirements rise modestly in older adults and during pregnancy and lactation, so anyone in those groups should check the full NIH ODS table or speak with a clinician.
Food sources such as fish, poultry, potatoes, starchy vegetables, and non-citrus fruits supply meaningful amounts of B6. High intakes from food alone have not been reported to cause harm, according to the NIH ODS.
The safety upper limit: where the two authorities disagree
This is the part of the guide that matters most if you are considering a B6 supplement.
The US Tolerable Upper Intake Level: 100 mg per day
The FNB set a Tolerable Upper Intake Level (UL) of 100 mg per day for all adults aged 19 and older. The NIH ODS states clearly that this limit applies to food and supplement intakes combined, not to supplements alone. Staying below 100 mg per day from all sources is the threshold the US framework considers unlikely to cause adverse effects in the general adult population.
EFSA's stricter limit: 12 mg per day
In 2023 the European Food Safety Authority (EFSA) reviewed the evidence and set a considerably lower upper limit of 12 mg per day for all adults, including during pregnancy and lactation. The gap between 12 mg and 100 mg is large, and it reflects genuine scientific uncertainty about where exactly the risk of nerve damage begins. Neither figure is a recommended intake target. Both are population-level ceilings, not personal prescriptions.
Why the nerve risk is taken seriously
Long-term high-dose supplementation with vitamin B6 can cause sensory neuropathy, a condition involving numbness, tingling, and pain, typically in the hands and feet. The NIH ODS attributes this risk specifically to prolonged supplementation at high doses, not to dietary intake. Because many B-complex and multivitamin products contain B6 well above the RDA, it is worth checking labels and adding up total daily intake from all supplements.

What the research says about supplementation
It is important to separate what vitamin B6 does as a nutrient from what supplementing with it has been shown to do in clinical research. The NIH ODS grades the evidence as follows:
- Strong evidence: Vitamin B6 acts as a coenzyme in protein and amino acid metabolism, and it helps maintain normal blood homocysteine levels.
- Moderate evidence: Vitamin B6 may help relieve nausea and vomiting in pregnancy. This is one of the more studied applications, though personal use during pregnancy should always involve a clinician.
- Limited evidence: Vitamin B6 supplements may reduce symptoms of premenstrual syndrome (PMS). The research exists but is not considered conclusive.
- Insufficient evidence: Supplemental vitamin B6, alone or combined with folate and B12, reducing cardiovascular disease or stroke risk; vitamin B6 supplementation preventing cancer or reducing cancer mortality; vitamin B6 improving cognitive function or preventing cognitive decline in older adults. The NIH ODS does not support these claims based on current research.
Honest grading of evidence is important. A nutrient can have genuine physiological roles and still lack proof that taking extra amounts in supplement form produces the outcomes people hope for.
Drug interactions to know about
Vitamin B6 interacts with several medications. Always talk to a doctor or pharmacist before combining any supplement with prescription drugs.
- Cycloserine (Seromycin): This antibiotic used for tuberculosis increases urinary excretion of pyridoxine and can worsen seizures and neurotoxicity. Pyridoxine supplements may help prevent these effects, but this should be managed by a prescribing clinician.
- Antiepileptic drugs including valproic acid, carbamazepine, and phenytoin: These drugs increase the breakdown of vitamin B6 vitamers, lowering plasma PLP and raising homocysteine. Separately, pyridoxine supplementation at 200 mg per day has been shown in some studies to reduce serum concentrations of phenytoin and phenobarbital, possibly by speeding their metabolism. This is a clinically significant interaction.
- Levetiracetam (Keppra): Preliminary evidence suggests vitamin B6 at doses of 50 to 100 mg per day in adults may reduce behavioral side effects such as irritability, agitation, and depression. This evidence is early and any use should be discussed with the prescribing physician.
